Please read and complete the below form to provide us with permission to take a photo and/or video of you.

This form relates to photographs, recordings or filmed footage of you which you agree to allow Electricity North West Limited to capture and use for public display such as online, commercial publications and non-commercial publications in any media. 

You agree that the media can be edited, modified, cropped and added to or subtracted from the original media at our discretion and without further approval from you. Any amends to the original media will not represent you in a derogatory manner. 

You also agree to allow us to use your name, likeness and any other biographical information provided by you in connection with the media, this would not include any of your contact details. 

By completing this form you agree and acknowledge that all right, title and ownership in and to the media captured of you, including copyright, shall belong to Electricity North West Limited.
